Every player on the roster needs the captain to confirm this on their behalf.
Assumption of risk. Five-a-side football involves physical contact and inherent risk of injury. By signing, the captain confirms that every listed player understands and accepts this risk.
Code of conduct. Players agree to compete under the official LHC rulebook, including the no-slide-tackling and no-tackling-from-behind rules, and accept the authority of The Critic (match officials) on the day.
Fair play. Any player receiving a red card is suspended for the remainder of that match, with the possibility of a further tournament ban at the organisers' discretion.
Image, likeness & media consent. Photos, video, audio, and livestreams captured at LHC events may be used by LHC — and, where relevant, its sponsors — across social media and marketing, including as short highlight clips ("Cook Moments"). This is non-exclusive and doesn't stop a player from being filmed by anyone else, and it doesn't hand LHC ownership of a player's image. A sponsor can't use footage to imply a player personally endorses their product. Appearing in this content doesn't by itself create any right to payment — any paid arrangement has to be agreed separately, in writing.
Your data (POPIA). The details on this form, together with any Content described above, are processed under the Protection of Personal Information Act 4 of 2013 for event administration, safeguarding, and the promotional use described here — nothing else. You can request access to, correction of, or deletion of this information at any time by contacting LHC.
Players under 18. If any player on this roster is under 18, a parent or legal guardian must have reviewed and separately agreed to this consent on that player's behalf before the captain signs below. LHC will not feature an under-18 player in sponsor content without a guardian's consent on file.
Withdrawing consent. This consent stays in effect until withdrawn. A player or guardian can withdraw it at any time by writing to LHC — future use stops immediately, though we can't guarantee removal of anything already published by LHC, a sponsor, or a third party.
